Structured Progression Pathway

Support that helps people move forward

At Northstar Care & Support, we offer an optional Structured Progression Pathway for individuals who want to actively work towards greater independence over time.
 

Our approach is not just about providing support in the moment. It is about helping people build confidence, develop life skills, create meaningful routines, and move forward in a way that is realistic, sustainable, and tailored to them.
 

This pathway is designed for individuals who want more than ongoing support alone. It offers a flexible, person-centred route that focuses on progress, personal growth, and long-term stability.

What makes this pathway different?

The Structured Progression Pathway is built around a simple belief:

Support should not only maintain — it should also help people progress.

This means working alongside each individual to identify goals, strengthen daily living skills, build confidence, and encourage greater independence over time.

The pathway is always shaped around the person, their circumstances, and their pace. There is no fixed timescale, no pressure, and no one-size-fits-all model.

How the pathway works

While every person’s journey is different, the pathway is typically built around four key stages:

1. Stabilisation

Creating a consistent and supportive foundation

The first stage focuses on building stability in day-to-day life. Many individuals we support may be experiencing low confidence, lack of routine, or feeling overwhelmed by everyday tasks.

During this stage, the focus is on creating structure, developing trust, and establishing a consistent support relationship. This may include introducing simple routines, encouraging engagement in daily activities, and gradually reducing feelings of isolation.

The aim is to help the individual feel more settled, supported, and in control of their immediate environment — creating a strong and reliable starting point for progress.

2. Development

Building confidence, skills, and engagement

Once a stable foundation is in place, the focus begins to shift towards personal growth and development.

This stage supports individuals in building confidence, developing practical life skills, and becoming more engaged with the world around them. This may include accessing community activities, exploring personal interests, improving social confidence, and working towards meaningful, person-led goals.

Support remains consistent, but becomes more focused on encouraging progress, participation, and a sense of achievement in everyday life.

3. Independence

Encouraging autonomy and self-direction

As confidence and capability increase, the focus moves towards strengthening independence.

This stage supports individuals in taking greater ownership of their daily lives, decisions, and routines. Support is still present, but is gradually adjusted to encourage self-direction, problem-solving, and increased autonomy.

The aim is not to remove support suddenly, but to reduce reliance in a safe, structured, and supportive way — allowing the individual to build confidence in managing life more independently.

4. Transition

Preparing for long-term stability and reduced support

For some individuals, the final stage focuses on preparing for a reduced level of support or a transition away from services.

This includes maintaining the progress that has been made, reinforcing routines and coping strategies, and supporting the individual to feel confident in sustaining their independence over time.

Where appropriate, this stage may also involve planning for the future, identifying ongoing support networks, and ensuring that the individual feels equipped, supported, and ready to move forward.

We recognise that, while stability is important, long-term support should also create opportunities for growth, confidence, and greater independence. Too often, support services can unintentionally maintain individuals at the same level, without creating a clear sense of progress or direction.

Our approach is different.

We work alongside each individual to build a strong foundation, develop practical life skills, and support gradual, realistic progress over time. This means focusing not only on immediate needs, but also on what the future can look like — always guided by the individual’s goals, choices, and pace.

Our Structured Progression Pathway reflects this belief. It offers a supportive and person-centred framework that encourages stability, builds confidence, and promotes independence, without pressure or unrealistic expectations.
